How To Get A Copy Of Your Ohio COVID-19 Vaccination Card!

As the omicron variant of COVID-19 continues to drive up cases and strain hospitals, vaccines continue to be touted as the number one tool to fight back. And the longer the pandemic goes on, and the more widespread vaccine requirements become, the more residents might have need to show a vaccine card.

Individuals receive a Centers for Disease Control and Prevention card when they get a vaccine dose. And while some places accept digital copies, the original version — or the equivalent proof of vaccination — is required elsewhere.

If an Ohioan needs their vaccination records, they have a few options, according to the state Department of Health:

  • Ask your doctor. They should have a copy of your medical records and access to the state system.
  • Contact your local health department. They should also have access to the state system.
  • Ask a workplace, camp or school that has your records.

If none of the above works, Ohioans can contact the Ohio Department of Health. Paperwork cannot be emailed or faxed to the department. Additionally, health department staffers cannot verify if your records are in the state system over the phone or via email.

Ohioans must instead mail a release authorization form with your original signature to the Ohio Department of Health. Your current mailing address must be included on the form and you must also include a photocopy of a government-issued ID with your signature, to be compared to the signature on the form.

If you need to request similar records for a spouse or family member, you’ll need to fill out a second release authorization form and provide their ID as well.
